more than 13 years experience with switching power supplies at two different companies in the greater toronto area. worked my way from production technician to sustaining engineering, then product development (electrical and embedded). also fluent in vba and labview. currently studying project management theory while working as a power supply designer.

A USB-sourced +5V rail is not going to be able to support high current without appropriate resistors on the +D and -D signals. I would check for input sags as well as the previous suggestions of appropriate input and output capacitors.

Also, it's generally not recommended to accept an answer quickly (even if it's right) since there's little incentive to add to the conversation once the points are awarded. 24 hours is a typical 'cooling off' period before accepting.

Nope. When the switch (SW) is off, D1 is conducting the freewheeling inductor current in the forward direction. The diode is chosen to specifically not break down due to reverse voltage, per Dave's answer.
